We are a nationwide company, with the sole purpose of establishing and running a robust palliative program at every local hospital.
Our goal and mission is to improve quality metrics, decrease length of stay, and save money for the hospital. This is achieved by us hiring a full-time practitioner, to be always at your hospital, with oversight by one of our physicians.
In our 15 years of experience, we have realized, that establishing a successful palliative care pro-gram requires a cultural shift amongst physicians and staff members. We achieve this by meeting with hospital leadership to identify key players who will help shift the culture by improving physician engagement and awareness, and that a program such as ours exists in your hospital and is available to benefit their patients.
Our practitioners will round with the multidisciplinary teams, whether it’s on the floors or in the ICU setting and will be available to provide consulting services.
We will also partner with local hospice companies, so the transition to inpatient hospice can be made quicker and seamless to improve the length of stay for the hospital. For patients that are not being transferred to an outside hospice facility, or to their home, our mission will be to transition them to inpatient hospice, to improve the mortality index for the hospital. By doing this, we expect the hospital to reduce their per-day cost significantly and improve their quality metrics and length of stay.

At Palliative & Hospice Consultants, our goal is to improve the quality of care for patients and families, while also helping hospitals curve cost. We work with insurance companies to bill and collect for our services, so there is no direct cost to the hospital. How we achieve this for YOUR hospital:
- We plan to implement the team with a palliative care trained physician, who will supervise a physician extender such as a practitioner or a physician’s assistant working alongside.
- The physician extender as well as the physician will need to be credentialed by the hospital, as per the medical staff bylaws.
- The physician extender would physically round in the ICU, as part of the multi-disciplinary ICU team every day and will be available to assist right away with any patient or family concern. The physician extender will also be seeing consults on the floors, when requested. Even though the NP will be on site, the physician supervision will be tele-based.
- The physician will have on-site visits initially every month to establish the program, in conjunction with the vision of the hospital. When the program is established, the physician on-site visits will be quarterly. or sooner if needed, to meet with key stakeholders and measure the quality of the program.
- The team would see consults on the medical floor and in the emergency room within 24 hours and communicate with the referring physicians the goals of care that are ex-pressed by the patients and their family members.
- The palliative care team will coordinate care with social workers, case managers, spiritual care staff, pharmacists, and consulting physicians.
- The team would also coordinate care with outpatient palliative care and hospice services, to enable a smooth transition of care.
- The team would have biweekly meetings with the Chief Medical Officer to discuss areas of potential improvement, address workflow, scheduling, and other concerns.
- The team would meet quarterly with referring providers to ensure that their goals are being met.
- The team would also participate in the Quality Committee meetings of the hospital.
